There was no process in place for identifying return oncology patients in need of diabetes
self-management education.
To increase the number of oncology patients identified in the Return Patient Clinic who needed Diabetic
Self- Management Education from approximately 10% to 60% by June 30, 2011.
Standard Work Instructions were developed to guide the dietitians' intervention for blood glucose management. A process for scheduling diabetes education follow up was implemented.
- We learned that it is important to scope the project so that it is manageable and realistic
- We learned the value of communicating with Stakeholders from other departments to help identify solutions to problems
- This project reinforced the important role that dietitians have in identifying oncology patients with comorbidities and providing interventions to improve their quality of care & quality of life
- Focus on quality lead to growth opportunities
- Many people neglected their diabetes management when being treated for cancer, in part due to lack of urgency from oncology providers
